Special education services (get IEP, 504 transferred or established) Make sure you have a copy to provide or the school will have you sign a release of information from the other school

Health Services Find a new primary care provider for your child and specialized services.
- Private Insurance – Call the number on the back of your insurance card or visit their website to find a provider covered by your insurance
- State Insurance – To apply for Washington State insurance visit https://www.wahealthplanfinder.org/
- Primary Care Provider (PCP) – Your insurance should have a list of providers that are covered under your insurance.
- Specialists/Therapy services – Insurance providers will be able to give you a list as well of providers they cover, you can also do a search online or if you’d like a list you may call or email us.
Social Services If you are receiving services in your current state you may need to transfer or reapply for some of these services:
